In qualifying cases, a temporary crown can also be placed the same day, so patients leave the appointment with a natural-looking tooth in place rather than a gap. The implant still requires the same osseointegration period as a traditional implant before the final crown is placed, but the overall treatment timeline is significantly shorter because two procedures are completed in one visit.

Is an Immediate Implant Right for You?

Not every patient is a candidate, and the evaluation Dr. Reich performs before recommending this approach is what makes it safe and predictable.

Immediate implants work best when the extraction site has sufficient healthy bone to anchor the implant securely, the surrounding gum tissue is in good condition, and there is no active infection present. Patients with a single failing or damaged tooth are often good candidates, as are those who have already been told they need an extraction and want to minimize the number of procedures involved. For patients who do not need an extraction, a single-tooth dental implant placed through the traditional staged approach may be the better fit.

Not every situation allows for immediate placement. Significant bone loss may require bone grafting or sinus elevation first, and active gum disease or certain systemic health conditions may also need to be addressed before an implant is placed. How the Procedure Works

Every immediate implant case at our practice follows a precise sequence that Dr. Reich reviews with you in full before anything begins.

1. Evaluation and Treatment Planning

Dr. Reich takes 3D imaging of the extraction site to assess bone volume, density, and the anatomy of the surrounding structures. This imaging is used to plan the exact position and angle of the implant before the procedure begins, so placement is guided and accurate rather than freehand. Written estimates are provided before any procedure begins.

2. Tooth Extraction

The failing tooth is carefully removed while preserving as much of the surrounding bone and gum tissue as possible. The goal is to leave the socket in the best condition for immediate implant placement.

3. Implant Placement

The titanium implant post is placed directly into the extraction socket in the same appointment. The implant must achieve sufficient primary stability in the bone for the immediate approach to proceed. If stability cannot be confirmed at the time of extraction, Dr. Reich will discuss your options, which may include a short healing period before placement.

4. Temporary Restoration if Applicable

For patients who qualify, a temporary crown is placed on the implant the same day. This temporary is designed for aesthetics during healing and is not intended for heavy chewing forces while osseointegration takes place.

5. Healing and Final Crown

Over the next 3 to 6 months, the implant fuses with the jawbone. Once osseointegration is complete, your general dentist places the permanent custom crown. Dr. Reich coordinates with your referring dentist throughout to ensure continuity of care. Patients who need a full-arch or All-on-4 solution follow a different process with a custom temporary delivered the morning after surgery.

If I need a bone graft, can I still get an immediate implant?

In some cases, a small amount of bone grafting material can be placed around the implant at the same appointment to fill gaps between the implant and the socket walls. This is different from a full bone grafting procedure and does not necessarily prevent an immediate implant from being placed. Dr. Reich will assess this at your consultation.

For qualifying patients, a temporary crown is placed the same day so you leave with something in place. Not every case allows for a same-day temporary, particularly if the implant site needs more time to stabilize, but Dr. Reich will tell you what to expect before your procedure begins.

 

You will receive detailed written post-operative instructions at your appointment. During the healing period, you will follow a soft diet and avoid putting heavy chewing pressure on the temporary crown. When the right patient is selected and the procedure is performed correctly, immediate implants have comparable long-term success rates to traditional implants. Failure risk increases when immediate implants are placed in sites with active infection, insufficient bone, or untreated gum disease.
